With recent advancement of three dimensional electroanatomical mapping system, functional property of the substrate has stepped into the spotlight, in addition to the anatomical reconstruction. Some new findings have overturned our conventional knowledge based on the surgical or traditional electrophysiology, and this area is still evolving. By endocardial and epicardial high density map, the detailed course of the circuit has been elucidated; relative slow conduction at the entrance and exit of the isthmus, unexpectedly high three-dimensional reentrant circuit in ischemic cardiomyopathy, and more frequent pseudo figure-of-eight reentrant circuit. We would like to share the novel findings and method using the new mapping systems.
